Patient
Information
New
Patients
New patients are
welcome with or without a referral from a general dentist. Your
first appointment will take roughly one hour during which we
discuss your dental goals and examine your mouth to give
recommendations as to the next step in achieving your dental
goals.
A second appointment involving radiographs (x-rays), impressions of
the mouth, detailed dental examination and photographs will be
needed to assess dental conditions and plan for treatment. At a
follow up consultation appointment an information booklet will be
provided to you outlining your personalized treatment plan and cost
estimate along with a time frame for completion of proposed
treatment.
We will provide you with a new patient information package before
your first scheduled appointment. It includes a medical history
form, dental history form as well as insurance information. If you
wish to save some time, you can download and print out our new
patient forms and fill them out ahead of time.

Office Hours
Our current office
hours are Monday thru Thursday by appointment only from 8 am to 5
pm, however Dr. Hopkins is available for emergencies. We invite our
patients to complimentary parking in any parking stall marked
“Reserved – Dr. Derek S. Hopkins Inc.”
Missed or Cancelled
Appointments
Your appointment time
is reserved only for you, thus a missed appointment means that
another patient is unable to be seen. If you must reschedule your
appointment, we ask that two-business days notice be given.
Payment Policy
There is no fee guide
for the specialty of prosthodontics in British Columbia, so Dr.
Hopkins has developed a fee schedule based upon clinic costs,
laboratory costs, and professional fees. The fees are inclusive,
however unforeseen costs such as the modification of treatment plan
during treatment, increases in laboratory fees, delays in
treatment, etc. may increase costs. Treatment costs and
options are always disclosed before any treatment commences.
For all examinations, consultations, and treatment performed,
payment is expected at the time the services are rendered or
initiated. Our office does not take assignment of dental
insurance plans, however we are happy to fill out your paperwork so
that you can be reimbursed for any coverage that your plan may
provide for dental services. We accept debit, cash, personal
cheques, Visa, and MasterCard payments.
Most insurance companies base their reimbursement on the General
Dentist Fee Guide, therefore the amount you receive may be lower
than the amount claimed.
